For the next four years, the Great Lakes Practice Transformation Network (GLPTN) will help you and 11,500 other clinicians prepare for anticipated industry changes calling for better care at lower costs.
Our networked, three-state coalition is dedicated to coaching you through the five phases of patient-centric practice transformation necessary to effectively participate in value-based payment systems.
You will be guided through CMS phases utilizing a Transformational Change Toolkit to help you achieve associated milestones as well as clinical operational results.
Eligibility: All providers and physician offices in IN, IL, MI who are not participating in an Accountable Care Organization (ACO) or other form of alternative payment model under CMS, and who are not enrolled in another Practice Transformation Network.
